1. Liability / Assumption of Risk (required) *

I acknowledge that volunteering at Shantivan Ashram involves physical activities (cooking, cleaning, gardening, lifting, outdoor work, etc.) that carry inherent risks of injury. I am volunteering of my own free will and assume full responsibility for my participation. I release Gayatri Pariwar Western Ontario (GPWO), Shantivan Ashram, their volunteers, organizers, and property owners from any and all liability arising from such participation, except in cases of gross negligence.

2. Medical Emergency Authorization (required) *

In the event of a medical emergency where I cannot communicate my wishes, I authorize GPWO staff and volunteers to obtain emergency medical care for me, including arranging transportation to a medical facility, administering basic first aid, and contacting emergency services. I understand I am financially responsible for any medical costs incurred.

3. No Compensation Acknowledgment (required) *

I understand that I am volunteering my time and labor without expectation of payment, wages, or any other form of compensation. I am not an employee of GPWO or Shantivan Ashram. Meals and (if applicable) on-site accommodation are provided as a courtesy, not as compensation.

4. Photo / Video Release (optional)

I grant GPWO permission to take photographs and videos of me during my volunteer service for use in promotional materials, social media, the GPWO website, and printed newsletters. (Uncheck only if you do NOT consent.)

5. Code of Conduct (required) *

I will follow the GPWO Code of Conduct during my service: respect for fellow volunteers, organizers, and the sacred space of Shantivan Ashram; abstaining from drugs, alcohol, smoking, and inappropriate language or behaviour; following all safety guidelines and instructions from coordinators. Serious or repeated violations may result in being asked to leave.

6. Safe Practices (required) *

I agree to follow all safety guidelines including: lifting with proper technique; wearing appropriate clothing/footwear for the task; reporting injuries immediately; not operating equipment I have not been trained on; respecting kitchen/food safety; staying within designated areas.
